The U.S. Wage and Tax Statement, commonly known as a W-2 form, is issued to employees before Jan. 31 showing the previous calendar year's wages and tax withholdings. Although a government form, employers complete the W-2 and are responsible for delivering the form to employees. Employees are responsible for reporting their wages and income tax withholdings on their federal and state income tax returns, and W-2 forms provide this information. Sometimes, W-2 forms are lost, misplaced, or never received. When this happens, employees must know how to request duplicate W-2 forms. There are several options available.
